When you call, tell them that you are buying a Distributor’s kit; and that you already have a sponsor. They
will ask you for your Sponsor’s name and Code number: My name is Armando Zarate, and my distributor

INSTRUCTIONS to fill out the membership contract
When you get the kit, fill out the membership contract (white page) , SIGN IT, and tear out the yellow page,
this will nbe your copy of the contract.
You will need to send the original contract (white page) by mail along with a picture ID to the Omilife
distribution center that sent you the kit.
The membership contract form is divided in three sections; the first section is for your information; the
second section is for a co-distributor info (optional).
The third section is for your sponsor info, (just write my name and distributor number, my signature is not
necessary.
if you need to place an order before you send the contract by mail, you can do so, by faxing a copy of your
contract, and a copy of your ID to the fax number printed in your INVOICE.
Wait 1 hour, and call 1-888-326 1188 to place your order, and confirm that they have received your
documents.
If you choose to send a copy of your contract by fax, you still have to send the original copy to Omnilife’s
corporate office (see address above) - NOTE: You do not have to fill out the W8 or W9 forms: they are for
taxes purposes only
IMPORTANT: The printed number on the application form, including your initials will be your
Distributor Number (write it down and memorize it).

There is no monthly minimum purchase required to keep your membership active. However,
you will need to accumulate a minimum of 1000 yearly points for your membership to
remain active.
You don't have to sell Omnilife products in order to get the membership, you can do it if you
want to, but Omniife does not force you.
If you decide to sell products, you can make a profit ranging from 20 to 40%. In addition, you
have the opportunity to create a network of distribution and consumption that will generate
royalties and commissions on purchases made by your downline.

POINTS AND DISCOUNTS:
Each product has a points value that "add up" to get you to the next discount level;
When you affiliate, you get an initial 20% discount;
When you accumulate 500 points you get a 25% discount.
When you accumulate 1000 points you get 30%, discount
When get to 2000 points you get a 35% discount.
To get to the maximum discount level of 40% you need to accumulate 1000 points for 4 consecutive
periods of 15 days. When you get to the 40% discount level you become a Bronze Level Distributor
or wholesale distributor.
